Wheaton Academy Placement and Recommendation Forms
Thank you for providing your input as a teacher, youth pastor, or mentor for a Wheaton Academy applicant. Your honest evaluation of this applicant will only be used for the purposes of admission and placement. Information given on these forms will be kept confidential and will remain in the student’s file during their enrollment at Wheaton Academy.
